వైద్య ప్రకటన
  1. I confirm that the health information I have given about my child is accurate and complete to the best of my knowledge, and I will tell the school promptly if it changes.
  2. I understand that school volunteers are not medical professionals and cannot administer medication to my child. If my child needs medication during class, they must be able to take it themselves, or I must make separate arrangements with the class teacher.
  3. I will not send my child to class with a communicable illness, and I will keep them at home until they are no longer contagious.
  4. If my child requires an inhaler, EpiPen, or similar device, I confirm that my child knows how to use it, that it will be with them during class, and that I have told the class teacher where it is kept.
  5. I understand that the school may give basic first aid to my child if it is needed.
  6. I understand that the Class Teacher may call for medical emergency help if they judge it necessary, and I accept that this is at my cost.
